I think this definitely betrays the influence of the moving timeline, i.e. the twins' delicacy occurs pre-antibiotics -- nowadays (and by the time of RAH, at least), medically fragile children are not as a rule held back from learning to swim. But it makes perfect sense for their time-of-origin.
